Maintaining one’s independence is important, especially as we age or become disabled. In today’s society, the demands of careers and maintaining a family can be challenging. Caring for an elderly or disabled person in addition to these demands can be stressful. The elderly, disabled and their family members are unaware of the services available to assistance in time of need.

The Partnership’s services for the elderly and disabled provide assistance to those individuals over 60 years old and the disabled age 18-59. The program is designed to provide assistance to individuals who require assistance with daily tasks to maintain their independence and provide advocates for individuals who are living in a long term health care facility. The Seniors and Persons with Disabilities staff will provide assistance with health services, in home services, public benefits, housing, and victims of abuse and violence, and advocacy services. The Seniors and Persons with Disability staff is committed to promoting independence and preserving the dignity of seniors and people with disabilities. Partnership serves people that reside in Hamilton county, northwest Georgia and Northeast Alabama.

SENIOR OUTREACH SERVICES
The Partnership's Senior Outreach Services offers comprehensive care-planning for older adults and their caregivers who need help finding the resources that will best address their unique aging-related challenges.

IN-HOME CARE
The Partnership's Homemakers assist elderly and disabled persons with the tasks of daily living, to allow them to continue independently in the comfort of their own home.

VICTIMS OF CRIME PROGRAM
The Partnership provides services to the elderly over age 60 and disabled persons age 18 – 59 in Hamilton County, who are victims of abuse, neglect, exploitation and criminal assault.

SERVICE COORDINATORS IN PUBLIC HOUSING
The Partnership’s Service Coordinators assists elderly and disabled residents of public housing to maintain an independent lifestyle by connecting them with community based services.

LONG TERM CARE OMBUDSMAN
The District Ombudsman and trained, Certified Volunteer Ombudsman Representatives (V.O.R.s) work to resolve problems that may arise in long-term-care facilities between elderly and disabled residents, their families and facility staff.
