Senior Outreach Services

The Partnership’s Senior Outreach Services program is a care planning resource for older adults and their caregivers who are confused by the complex maze of services needed to solve aging-related problems.  You trust your financial planner to help solve your money problems.  You can likewise trust the Partnership to help you or those in your care find improved health, independence and fulfillment in aging.

It’s a simple process.  Once you contact us, we ask you to fill out a brief application that helps us determine if care planning is right for you or your loved one.  We then pay you a personal visit in which we assess the problems that need professional attention.

We then conduct extensive research to develop a care plan designed to address the your unique challenges.  It summarizes the problems you are experiencing, suggests appropriate interventions, and recommends reputable, qualified providers who can help you out.  All done within a budget that you specify.

That’s it!  Once we create the plan, it’s yours to implement at a pace that makes you most comfortable.  It’s all the solutions you need right at your fingertips and in your control.  It’s peace of mind, and it’s affordable.  Best of all, it comes to you from the Partnership—your trusted resource for elder services.
